What happens to a coal cellar once it is finished?

It gets a second wall built in front of the first one. The usual job puts furring strips, batt insulation and drywall or paneling straight against rubble stone or early poured concrete that has always passed a little moisture through its joints. Nothing about the water changes. What changes is that the moisture now arrives inside a closed cavity, in the dark, against paper-faced drywall, with no air moving through it. Carpet laid on the slab does the same trick from below.

Two other habits of the era compound it. The rim joist above the foundation is often packed with fiberglass, which holds humid air against cold wood at the exact point where the house meets the ground. And the perimeter drainage, if the house ever had any, is now roughly a century old and full of silt.

This is why a finished basement is the one part of an Arlington house that genuinely benefits from being read with a meter. From the room side, a wall soaking wet behind the drywall looks exactly like a wall that is fine.

Why do the symptoms show up once the heating comes on?

Because the house closes up and the basement air starts circulating through all of it. Most of these homes have been tightened over the years with replacement windows, attic insulation and air sealing, all of which is worth doing. It also means that from the first cold week onward, the same air moves between a finished basement and the rooms above through the ducts, the stair and every gap around a pipe. If something is growing behind that basement finish, the whole house shares it.

Whether a cough or a stuffy head is coming from the building is a question for a doctor, not for a web page. What sampling the air can answer is the other half of it: whether the spore load indoors is genuinely raised, which tells you whether to keep looking at the house or leave it alone.

The low side of town deserves a separate note. Along Mill Brook and out toward the Alewife floodplain the ground holds water for long stretches, so a foundation there is dealing with a permanent condition rather than an event. Spending on drainage, a sump that works and finishes that can get wet without feeding anything beats another round of sealant on the inside face of the wall.
